Please complete the New Employee Checklist and return with required forms and all necessary supporting documents.

• W-4 Federal Income Tax Withholding
• M-4 State Income Tax Withholding (Only required if different from Federal Withholding)
• I-9 Employment Eligibility Verification (Must include supporting documentation-see list on page 3)
• Direct Deposit Form (Mandatory)
• SSA Form 1945 regarding Social Security Benefits 
• OBRA Government 457(b) SMART Plan (Mandatory)
• Beneficiary Designation Request Form (Required for Police and Fire ONLY)
