Obtaining certified copies of a Death Certificates
The Freetown Town Clerk's Office maintains the Death Certificate records of individuals that were residents at the time of their death or of individuals whose death occurred in Freetown (resident and non-resident).
To obtain a certified copy (1) of a death record, please mail the request including the name and date of birth, a check in the amount of $10.00 made payable to the Town of Freetown and a stamped, self-addressed envelope to: Town Clerk, P O Box 438, Assonet, MA 02702. We will mail back the certified copy of the record the day we receive the request and check.
NOTE: Please call the Town Clerk's Office BEFORE making online payment/request, if you are not sure that the record is on file in Freetown. 508-644-2201 ext 2
